New Patients

If you are visiting our office for the first time then you can make a request for an appointment. Once you have registered you will receive and email with in (3) working days that will provide you with your portal access password.

Current Patients

If you are currently a patient, you must first complete your portal registration module then you can use this website for requesting a new appointment, request a refill etc. at your convenience.

Messages related to acute and or urgent matters should not be placed in the portal.  Patients should contact the office via phone / or the answering service after hours.  If this is an emergency please call 911.
